Purpose of the form -"European Accident Report" is getting an insight into facts surrounding the accident based on a mutual agreement between each driver. Continental driver will have a similar form in a number of languages and it does not matter whose form will be filled in, but it is important to make sure that you have kept one copy for yourself that you will later send to your insurer. (e.g. French person will fill in its section of the form in French, and will give you to fill in your part on his/her form in Bosnian language - you will understand the questions by comparing them to your form).

1. Mark down information for all witnesses before they leave the scene. Fill in question number 5;
2. If possible use a pen and fully fill in either blue or yellow part of the Motor Vehicle Accident Form (you need to state data from your insurance policy, green card and drivers licence);
3. Once you consider your statement duly filled in, sign it and pass on to the second driver for signature (15);
4. Do not forget:
   - item 10 - clearly mark the collision spot;
   - with "x" mark the option that relates to your accidents (item 12) and finally state the total number of "x"-marked fields;
   - sketch the accident (13) marking all data listed.

b) At home

1. Fully fill in Motor Vehicle Accident Report at the back of the Bosnian copy of the form.
2. Immediately send duly filled in forms to your insureer.

This form can be used even if no other vehicle was involved in the accident. E.g. damage caused by yourself, theft, fire, accident involving a pedestrian etc.

Unauthorised copying of the form is not allowed without prior written agreement of the European Insurance Committee that is the copyright owner. Any exchange or change to this document without prior authorisation of the European Insurance Committee can lead to court prosecution.
